Consent

Education Queensland, on behalf of the Ministerial Council of Education, Employment, Training and Youth Affairs (MCEETYA) manages the Bullying. No Way! website as part of the Safe and Supportive School Environments: Finding workable solutions for countering bullying, harassment and violence in schools Working Group. Education Queensland would like to use works created by others for the purposes of the Working Group and the Bullying. No Way! Website.

This Consent Form gives Education Queensland permission to use created works (the Works) for purposes outlined below. It does not mean that the ownership of copyright in that Work is transferred - simply that Education Queensland has permission to use the Works for the purposes mentioned.

1. These Works might include my written work (eg stories and poems), paintings, pictures, drawings, designs, photographs, videos, films, music, performance, computer programmes, web sites, sculptures, fashion or costume, metal or wood works or any other works I create.
2. I give permission to Education Queensland to use the Works for:
   • the Bullying. No way! Website
   • any purpose related to the Safe and Supportive School Environments: Finding workable solutions for countering bullying, harassment and violence in schools project
   • future media activities which would assist with further MCEETYA curriculum requirements
   • promoting and advertising of the Bullying. No way! website and other MCEETYA purposes; and
   • any related commercial purpose.
3. I will not withdraw the permission I have given on the basis that Education Queensland understands that I may choose to give this permission to other people.
4. I understand that by giving this permission, Education Queensland can use the Works in any way it chooses, for the purposes described above. It may reproduce them in any form, in whole or in part, edit or amend them as seen fit and distribute them by any medium including the Internet, CD-Rom, or other multi-media uses.
5. I give permission to non-profit educational institutions to use the Works for an educational purpose of that institution.
6. I understand that I will not be paid by Education Queensland for giving this permission.
7. I permit Education Queensland to disclose the following information about me when using or reproducing the Works:
   • Alias
   • Name
   • Age
   • School
   • State/Territory
8. I agree that that the Works are my original work and do not infringe the copyright of anyone.
9. I warrant that
   1. I am over 18; or
   2. I am under 18, and I have gained permission from my parent/guardian to contribute this Work; or
   3. The Work was created as part of my work duties, and I have gained consent from my employer to contribute this Work.
10. I understand that a representative of Education Queensland may contact my parent/guardian/employer to confirm their consent.
